Student Emergency and Health Information Form |
The Student Emergency & Health Information Form should be completed ANNUALLY for ALL students attending St. Mary's School.
It replaces the Emergency Information Card & Health History Information sheet that was used in past years. It hopefully will help cut down on the amount of paperwork you need to return to us.
This is a very important document that informs us how to contact you in the event of an emergency at school or if your child should become ill or injured while in our care. It also alerts us to any health conditions your child may have.
Please make sure to keep our records current by sending in writing any changes in
contact information that may occur during the school year.
Please also alert us in writing if your child develops any new medical conditions or if medications change during the school year.
Our records can only be as accurate as the information we receive.
Keeping up to date information will help us to better serve you and your child.
There are two different versions of this form for your convenience.
The “Fill In” form allows you to type in the requested information in the fields provided and then print it when you are finished.
If you would prefer to print this form first and write in the information, you need to open the “Print First and Write” version of this form.
Either form is acceptable and should be sent to St. Mary's School Nurse once completed.
